As an initiative towards the management of obesity in the community ,   Centre Health Complex is introducing a Weight Management Clinic. This clinic will provide the clients  an opportunity to set their goals for weight management and will facilitate an active lifestyle with  a favorable outcome for prevention of chronic diseases.

Your GP will work out a care plan that is best suited to your needs in regards to managing your health needs. Once your Enhanced Primary Care plan is in place, you will be able to access eligible allied health and/or dental care services identified in the plan, on referral from your GP.

As Centre Health Complex already has EPC allied practitioners on site, implementing your new care plan is as easy as asking the receptionist to book your initial appointment for you.

For your first visit with your allied health practitioner you will need to bring the EPC form which has been signed and given to you by your GP and the referral from your GP. For any consecutive visits your information will be kept on file so you will not be required to bring your referral each time.
